SSOA Soccer officials please use the form link below to notify the Soccer Regional Supervisor Of Officials of any disqualifications or ejections during the season. Please remember that ALL disqualification/ejection reports must be submitted ASAP but no later than 24 hours after the occurance. Please consult with your supervisor as they may have specific procedures for disqualification/ejection reports.

*Submitting the disqualification/ejection information to the Soccer Regional Supervisor Of Officials DOES NOT satisfy the requirement of submitting an online disqualification/ejection report to the NCHSAA. Be sure you do both!

The NCHSAA DQ/ejection form must electronically submitted, or faxed to the NCHSAA within 24 hours or 1 working day following the completion of the game. In accordance with NCHSAA HANDBOOK policy, a fine of FIFTY DOLLARS ($50.00) will be imposed for failure to submit this report to the NCHSAA by the deadline.
